DISEASE MANAGEMENT PROGRAMS

CAC-Florida Medical Centers' philosophy is that physicians and patients must work as a team in the prevention and management of disease. CAC strives to educate its patients about their disease conditions, management, complications and how to prevent them, and provides patients with the tools and resources to participate in the management of their conditions.

“Be in the Driver's Seat with Diabetes” - a Disease Education and Management Program for Diabetics

CAC aims to improve patients' understanding of their disease and how to be in control of their diabetes.

All Medicare diabetic patients are invited to participate in this program, which consists of a series of educational lectures and brochures, in which the patients receive information regarding diabetes and accompanying conditions. Instruction is also given on how to prevent, identify and treat the complications of this disease. Patients are given guides to assist them in monitoring and managing their condition. Nutritional counseling and exercise classes in the Wellness and Activities Centers help to promote healthy life-style changes for better outcomes.

Physicians also receive information that assists them in reminding their diabetic patients about routine preventive check-ups and care, and to comply with the guidelines established by the American Diabetes Association.

CAC-Florida Medical Centers has received three-year recognition from the National Committee for Quality Assurance (NCQA) and the American Diabetes Association Diabetes Physician Recognition Program. CONGESTIVE HEART FAILURE CLINIC AND PROGRAM

Congestive Heart Failure or CHF is a common diagnosis in the elderly. In fact, it is one of the most common reasons for admission to the hospital in this population. The treatment of CHF requires that the patient follow a diet and take medication regularly, and be aware of early symptoms of worsening of their condition. CAC-Florida Medical Centers started its CHF Program and opened its CHF clinics so that these patients would have less hospital admissions and a better quality of life.

In addition to regular primary care physician visits, patients with CHF are also seen by the CHF Clinic team. This team consists of specialists in Internal Medicine and Cardiology, nurses, nutritionists, pharmacists and patient liaisons, who together provide treatment, education, support and continued care. The patient and the patient's family when appropriate are given the education and resources to take a central role in the self-management of the disease.

CAC's CHF Program consists of:
  • Visits to the CHF Clinic physicians
  • Individual or group nutritional counseling
  • Individual or group consultation with a pharmacist
  • Evaluation of social needs
  • Telephone calls from the CHF nurse to evaluate your progress, and provide education and support
  • Referrals to community resources
  • Educational materials about CHF, its symptoms and treatment

PHYSICIAN HOUSE CALLS

CAC's mission is to improve the quality of life of our patients through accessible, comprehensive and quality health care provided in a warm and compassionate setting. CAC provides physician services in the comfort of your home for non-emergency care, including lab and diagnostic tests. This service is offered to our established Medicare patients, who have certain medical and other conditions. The physician house calls are scheduled by CAC.
