CAC-Florida Medical Centers' philosophy is that physicians and patients must work as a team in the prevention and management of disease. CAC strives to educate its patients about their disease conditions, management, complications and how to prevent them, and provides patients with the tools and resources to participate in the management of their conditions.
“Be in the Driver's Seat with Diabetes” - a Disease Education and Management Program for Diabetics
CAC aims to improve patients' understanding of their disease and how to be in control of their diabetes.
All Medicare diabetic patients are invited to participate in this program, which consists of a series of educational lectures and brochures, in which the patients receive information regarding diabetes and accompanying conditions. Instruction is also given on how to prevent, identify and treat the complications of this disease. Patients are given guides to assist them in monitoring and managing their condition. Nutritional counseling and exercise classes in the Wellness and Activities Centers help to promote healthy life-style changes for better outcomes.
Physicians also receive information that assists them in reminding their diabetic patients about routine preventive check-ups and care, and to comply with the guidelines established by the American Diabetes Association.
CAC-Florida Medical Centers has received three-year recognition from the
National Committee for Quality Assurance (NCQA) and the American Diabetes Association Diabetes Physician Recognition Program. We are very proud of this achievement because it recognizes physicians who have met standards demonstrating delivery of medical care of the highest quality to patients with Diabetes.
